Teacher-Child Ratio — , on 4/23/2024 at 3pm, there 14 preschool children, 9 toddlers and 2 (S1 and S2) Teachers. On 5/10/2024 at 3pm, Hele Qiu stated that she was on break and S3 was supervising the toddlers with S2. S2 and S3 are Teacher Aide's. This poses an immediate risk to the health, safety or personal rights to children in care.

Sign In and Sign Out — , As a result of a Complaint Investigation dated 05/10/2024 at 11 am, LPA asked Helen Qiu to provide Children's sign-in and sign-out sheets with the signatures required by Section 101229.1 records from the Month of April 2024. Helen Qiu stated that she does not have the record at this time and will provide LPA later that day which poses a potential risk to the health, safety, or personal rights of children in care.
Personal Rights Section 101223 (a)(3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse, or other actions of a punitive nature........ Submitted evidence showed that during snack time (date unknown), S1 pulled away C1’s right arm in a fast and abrupt manner when C1 was trying to reach an item that appeared to be a snack on the table and S1 picked up the item fed it to C1 in a fast and abrupt manner which is a POTENTIAL risk to the health, safety, or personal rights of children in care. — requirement not met
