Child's Medical Assessments — , Based on the interviews and reviewed records that LPA conducted, prior to Child 1 (C1) admission, the licensee was notified by C1’s parent, of C1’s aggressive behaviors, including but not limited to, hitting and biting towards other children in care from C1’s previous childcare providers. The licensee failed to obtain C1’s medical assessment to assess whether the center can provide necessary health-related services to the child. This resulted in C1’s reported multiple biting incidents towards C2 and rmultiple reported aggressive behaviors incidents (hitting) towards staff members. This poses a potential risk to health, safety or personal rights to children in care.
Health-Related Services — the following, based on the LPA’s conducted interviews, and reviewed facility records that child 1 (C1) had multiple undocumented incidents involving other children in care and staff members. LPA reviewed and a obtained a copy of a partially completed LIC 624 Unusual Incident / Injury Report, completed on 6/4/2026 with an incident date of 6/23/2026. The incident involved C1 bit Child 2 (C2) on the right shoulder during an outdoor play, approximately around 4:30 pm. C2 received a bite mark with an open skin, the size of less than half inch. This poses a potential risk to the health and safety, of the children in care.
