Infant Safe Sleep — Based on interview, licensee had not enabled the crib for the one infant in care and stated that she was using a cushion and not using the crib for infant sleep, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Records — Based on record review and interview, the licensee did not complywith the section cited above, as the licensee did not have personnel files for her adult daughters Silvana Hammo and Steva Hammo, who she identified as helpers, which poses/posed a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on record review, licensee did not have immunization records in the case of 3 of 3 children in care, children C1, C2, and C3, which poses/posed a potential health, safety or personal rights risk to persons in care.
