Mandated reporter training — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During staff file review, the LPA was unable to review a valid Mandated Reporter for S1, S2 or S3.
Family home — preventive health training, including pediatric CPR and first aid — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During staff file review, the LPA was unable to review a valid CPR/1st Aid card for S1, S2 or S3.
Staff immunization — influenza, pertussis and measles — Based on observation, interview, record review, the licensee did not comply with the section cited above in one of two staff present did not have documentation of TB screening and Pertussis (TDap) on file, which poses a potential health, safety or personal rights risk to persons in care.
Operating without a license — During the facility tour the LPA observed Trader Joes 2% milk in the fridge. The Licensee stated she provides the 2% milk to the children in care. Licensee stated per the food program the children must be given 2% milk. After reviewing the food program guidelines with the Licensee, the guidelines state, “1% milk…”.
Mandated reporter training — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review of S2 the LPA was unable to review a current Mandated Reporter certificate for S2. Licensee understands Mandated Reporter training must be completed every two years.
