Napping Equipment — Based on observation, the facility did not comply with the section cited above, LPAs observed class 02, two children were nappng without a cover on the napping equipment which poses a potential health, safety risk to persons in care.
Activities/Napping — Based on observation, the facility did not comply with the section cited above LPAs observed a class were supervising 16 children during nap time. LPA observed 5 children were awake and 11 children were napping which poses a potential health, safety risk to persons in care.
Written directives for drinking water lead testing — record review. Lead test results showed that three water sources had an action level exceedance. LPA observed all three outlets had been replaced with new fixture.This is a potential risk to the health and safety of the children in care.
