(e) All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing or volunteering in a licensed facility. This was not met as evidenced by: During a tour of the facility, LPA determined S1 does not have a criminal record clearance. This poses an immediate threat to the health and safety to the persons in care.

Disaster and Mass Casualty Plan — Based on record review, the licensee did not comply with the section cited above. Director stated they have not done a disaster drill yet, and was unable to provide proof of one done in the last 6 months. This poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on LPA's observations, the licensee did not comply with the section cited above. The kitchen has 2 cabinets that hold hazardous chemicals with no lock. The kitchen door always remains open, for ventilation purposes, children do access the area near the kitchen door. Furthermore, there is a drawer with kitchen knives also accessible. This poses/posed a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above. The outdoor drinking water fountain, has two wooden pieces that the fountain is bolted on. One out of the two pieces has completely worn down, exposing sharp pieces of wood. The water fountain itself works, and has collected dirt and leaves on it, as well as calcium from water. This poses/posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above. 3 out of 3 teachers files did not have an updated Mandated Reporter Training (MRT) on file for "Child Care Providers". S1, S3 and S4 did not have MRT on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above. 1 out of 3 teachers files did not have immunizations on file. S3 did not have immunizations on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Health-Related Services — Based on observation, record review and directors own admission, the licensee did not comply with the section cited above. There was no proof of written approval and instructions from the child's authorized representative for the administration of the medication to the child. This poses/posed a potential health, safety or personal rights risk to persons in care.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This was not met as evidenced by: During a tour of the center, LPA noted areas of the center not being used with signs of rodents. Additionally observed were cobwebs, dust, debris, rugs, large toys, and stacks of paper piled on top of each other. LPA noted a foul odor in one of the on-limits classrooms, & unable to determine its source.

(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors(1)The licensee shall take measures to keep the center free of flies, other insects, and rodents. This was not met as evidenced by: During a tour of the center, LPA noted areas of the center not being used with signs of rodents. Pertinent parties stated on/about 08/18/25, staff were instructed to sweep up rat droppings found in a first-floor storage closet and additional droppings on the second floor.LPA reviewed a photo of a

Drinking water lead testing — Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed lead testing was not conducted before 1/1/23

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA observed two staff did not have their mandated reporter training.
