Large family home licensing requirements — Based on the interview and observation, the Licensee did not meet the above regulation which poses an immediate risk to the children in care. During the facility tour, S3 attempted to test two smoke detectors, however, they were not in working condition. The Licensee stated the batteries are dead.

During the facility tour LPA observed a knife in the sink. S2 observed and agreed that the knife was accessible to the tallest child in care.
Staffing Ratio and Capacity — Based on the observation, the Licensee did not meet the above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. Upon arrival LPA observed S2 alone with nine children. S2 stated the Licensee does school drop offs in the morning and was not present at the facility. Licensee arrived at the facility 10-15 minutes after the inspection started.

Mandated reporter training — Based on the record review, the Licensee did not meet the above regulation which poses a potential Personal Rights risk to the children in care. During the review of staff files the LPA was unable to review a Mandated Reporter for S1 and S2 who was providing Care & Supervision during inspection.
Immunizations — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review, the LPA was unable to review immunizations for C3.
Immunizations — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review, the LPA was unable to review a completed CDPH 286 for all children reviewed.
Infant Safe Sleep — Based on the record review, the Licensee did not meet the above regulation which poses a immediate/potential safety risk to the children in care. During the children’s file review the LPA reviewed sleep logs for C2, C3, C4 that were timed for every 30 minutes. All sleep logs had set times such as 12:15PM, 12:45PM, 1:15PM, etc.

During the review of staff files the LPA was unable to review a Mandated Reporter for S3 who was providing Care & Supervision during inspection.
Staff immunization — influenza, pertussis and measles — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health to the children in care. During the review of staff files the LPA was unable to review immunization records with MMR.
Operation of a Family Child Care Home — Based on the record review, the Licensee did not meet the above regulation which poses an potential personal rights risk to the children in care. During the review of childrens files the LPA was unable to review an LIC 282 for C1.

Infant Safe Sleep — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA was unable to review current sleep logs for two infants.
Mandated reporter training — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA was unable to review current Mandated Reporter certificates for herself and S2.
Family home — preventive health training, including pediatric CPR and first aid — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA was unable to review current CPR/1st Aid certificates for herself and S2.
Alterations to Existing Building or Grounds — Based on the observation the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During facility tour LPA observed an off limits area (Laundry Room) not made inaccesble to children in care. LPA observed the door to be open.
Operation of a Family Child Care Home — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA was unable to review an LIC 282 for six of nine children present at the facility.
