Buildings and Grounds — Based on LPA observation on 02/11/2026, All the fire extinguishers in the facility expired on 12/20/2024 which poses an immediate health, safety or personal rights risk to persons in care.

Food Service — The facility kitchen does not have hot water for the washing and/or drying cycle to ensure that dishes and utensils are cleaned and sanitized which poses an immediate health, safety or personal rights risk to persons in care.

Personal Rights — LPA observed there is no heat in the preschool classrooms. Children must wear their sweaters or jackets to keep warm. This poses an immediate health and safety risk to children in care.

Buildings and Grounds — There is no working heater in the preschool class rooms. This poses an immediate health and safety risk to children in care.

Buildings and Grounds — LPA observed in the preschool classrooms Paint is chipping off all the walls, all baseboards need cleaning/fixed, windows need coverings and or curtains cleaned or replaced. All window seals need cleaning, ceiling air venst need cleaning, several outlet covers missing which poses/posed a potential health, safety or personal rights risk to persons in care.

Indoor Activity Space — LPA observed in the preschool classroom carpets, floors including all corners need cleaning, removal of all wax buildup and missing floor tiltes which poses/posed a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — LPA observed in the preschool bathroom 2 toilets and 2 sinks not working, all sinks do not have hot water or knobs for both. Furniture need painted/cleaned which poses/posed a potential health, safety or personal right risk to person in care.

Outdoor Activity Space: The playground shall be enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence shall be at least four feet high. LPA observed in the preschool outdoor play yard it is not enclosed near the school age walk way and the back outdoor gate leading to the street is accessible for a preschool child to go out of, which poses/posed a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on LPAs observations and photographs obtained, the facility did not comply with the section cited above as evidenced by the cushion padded titles under the play structure and swings in the preschool yard are loose and expose the concrete, which poses a potential risk of the health, safety, and personal rights of children in care.

Buildings and Grounds — Based on LPAs observations and photographs obtained, the facility did not comply with the section cited above as evidenced by LPAs observed varies sections of the concrete floor were broken and cracked between the play structure and swings which poses a potential risk of the health, safety, and personal rights of children in care.

Personnel Requirements — Based on (record review), the licensee did not comply with the section cited above, staff #3 has an incomplete health screening report which poses/posed a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation, the licensee did not comply with the section cited above the surface in the outdoor playground under the swings are not in safe condition, the cushion material to absorb a fall are loose and expose the concrete. In addition classroom #1 has two broken windows that are covered with construction paper which poses/posed a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on (record review), the licensee did not comply with the section cited above as child #2 did not have immunizations on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Written directives for drinking water lead testing — Based on record review, the licensee did not comply with the directive above, as eight outlets (outlets used for hand washing) out of twenty six outlets tested with an Action Level Exceedance (ALE). This poses a potential Health and Safety risk to children in care.
