Buildings and Grounds — Based on LPA observation on 02/11/2026, All the fire extinguishers in the facility expired on 12/20/2024 which poses an immediate health, safety or personal rights risk to persons in care.

Food Service — The facility kitchen does not have hot water for the washing and/or drying cycle to ensure that dishes and utensils are cleaned and sanitized which poses an immediate health, safety or personal rights risk to persons in care.

Personal Rights — LPA observed there is no heat in the Infant classrooms. Children must wear their sweaters or jackets to keep warm. This poses an immediate health and safety risk to children in care.

Buildings and Grounds — There is no working heater in the Infant class rooms. This poses an immediate health and safety risk to children in care.

Buildings and Grounds — LPA observed in the infant classrooms Paint is chipping off all the walls, all baseboards need cleaning/fixed, windows need coverings and or curtains cleaned or replaced. All window seals need cleaning, ceiling air vents need cleaning, several outlet covers missing which poses/posed a potential health, safety or personal rights risk to persons in care.

Indoor Activity Space — LPA observed in the infant classrooms carpets, floors including all corners need cleaning, removal of all wax buildup and missing floor tiltes, LPA smelled a musty/mold scent in the infant class room next to the preschool bathroom that flooded on 01/23/2026 which poses/posed a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — LPA observed the sinks does not have hot waterand is dirty, furniture need painted/cleaned and the changing and is very dirty which poses/posed a potential health, safety or personal right risk to person in care.

Buildings and Grounds — LPA observed cat feces on the fake turf on the infant yard, the infants had not gone out to the yard yet and staff had not skimmed the yard, the side yard next to the infant class rooms is dirty and the exit door paths need to be cleaned and cleared to open properly and safely. which poses/posed a potential health, safety or personal right risk to person in care.

Buildings and Grounds:(a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. This requirement was not met: Based on LPAs observations and photographs obtained, the facility did not comply with the section cited above as evidenced by broken stucco exposing the concrete on the lower portion of the north side of the wall between the entrance of the yard and barricaded AC unit in the infant play yard, debris of leaves outside of the infant classroom #1 and in the play yard, which poses a potential risk of the health, safety, and personal rights of children in care.

Outdoor Activity Space (1) Provide a shaded rest area for the children. This requirement was not met: Based on LPAs observations and photographs obtained, the facility did not comply with the section cited above as evidenced by the infant play yard did not provide shade to accomodate for the weather.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 3 out of 3 staff file review did not have complete immunizations on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Assistant Infant Care Center Director Qualifications and Duties — Based on (record review), the licensee did not comply with the section cited above in one out of one did not complete the Oreintation Step 1 training which poses/posed a potential health, safety or personal rights risk to persons in care.

Child Care Center Director Qualifications and Duties — Based on (record review), the licensee did not comply with the section cited above in one out of one, assistant director did not complete Preventative Health & Safety training which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Records — Based on record review, the licensee did not comply with the section cited above in two out four children are missing physicans reports which poses/posed a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on record review, the licensee did not comply with the section cited above in three out of four children did not have immunization records in the file which poses/posed a potential health, safety or personal rights risk to persons in care.
