LPAs observed that the electrical box located in the infant outdoor play area is accessible to children due to unlocked door. Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above. Staff Jennifer Foster is missing a copy of immunization record on file. This poses a potential health, safety or personal rights risk to persons in care.
Staff-Infant Ratio — Based on interviews conducted the licensee did not comply with the above regulation as there have been multiple days that both infant classes are out of ratio in the early morning. This poses a potential risk to the health and safety of children in care. This is a repeat violation and civil penalty of $250 is being assessed today.
Infant Care Food Service — Based on interviews conducted with staff and parents. The children's zippy cups are kept at the facility in the older infant classroom and it does not get return to the authorized representative at the end of the day.
Health-Related Services — Per interviews conducted there have been incidents where a child sustained a bite and injury and the authorized representative is not informed of the nature of the incident during pick up. This poses a potential health and safety risk to children in care.
Personal Rights — Per interviews conducted with staff members, day care parents, the facility serves children choking hazards food including raisins and craisins. This poses a potential health and safety risk to children in care.
