Teacher-Child Ratio — Based on LPA observation on October 2, 2023 between 2:30-2:45 p.m. the facility failed to meet the teacher to child ratio when one teacher was supervising 15 children. This is an immediate health and safety risk to the persons.

Outdoor Activity Space — Based on observation, there were paint chips in the sand and four pieces of broken glass in the outdoor activity area. The items were accessible therefore a hazard was present, which poses a potential health and safety risk to the children in care. A civil penalty of $250 was assessed.

The items were accessible and are not age appropriate therefore a hazard was present, which poses a potential health and safety risk to the children in care.
Responsibility for Providing Care and Supervision — Based on interviews and record review, care and supervision was not provided to meet the needs of C1 when C1 was involved in seven biting incidents between August and October 2023. In addition, an assistant was left alone in the classroom for 10 -15 minutes, which poses a potential Health, Safety or Personal Rights risk to the persons in care.
Personal Rights — Based on observation and record review, the licensee did not ensure each child was accorded a safe and healthful environment when S1 did not wear a mask in the Center for 10 days following exposure, which poses a potential Health, Safety or Personal Rights risk to the persons in care.
Reporting Requirements — Based on interviews and records reviewed, the Center failed to report C1 biting others on five separate dates. C1 was suspended for the behavior. This unusual incident threatened the physical or emotional health of C1 or the other children in care, which posed a potential Health, Safety, and Personal Rights risk to persons in care.
