Reporting Requirements — Based on interviews and records review, the Center failed to report an unusual incident involving C1 being shown a weapon at the Center. This incident threatened the physical or emotional health of C1, which posed a potential Health or Safety risk to persons in care.
Transportation — Based on observation and interviews, the licensee did not comply with the section cited above when one of the light bulbs on the rear driver side of the facility van was broken. Per facility representative the van is used daily to pick up the school age children. This poses/posed a potential health, safety or personal rights risk to persons in care.
