Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above as the playground area contained exposed material on the ground and a nail sticking out of the play structure post, which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above as two parent volunteers were missing documentation of one or more immunizations which poses a potential health, safety or personal rights risk to persons in care.
