Staffing Ratio and Capacity — During observations while conducting a tour of the home, the facility was found to be over capacity with 26 children in attendance on this date which poses an immediate risk to health, safety, and personal rights to children in care.

Section 102416 - Personnel Requirements: (d) Prior to employment or initial presence in the child care home, all employees & volunteers subject to a criminal record review shall: (1) Obtain a California clearance or a criminal record exemption. This requirment was not me as evidenced by: During record review, four (4) staff members present in the child care home today were found to not have a criminal record clearance which poses an immediate risk to the health, safety, & personal rights to children in care.

Staffing Ratio and Capacity — During observations and record review, LPAs found that there are 9 infants and 17 older children present today which poses an immediate risk to the health, safety, and personal rights to children in care.

Family home — children supervised at all times — During observations and interviews with staff, the licensee was found to be absent from the home for a time that exceeded 20 percent of the hours of operating hours which poses a potential risk to the health, safety, & personal rights to children in care.

Alterations to Existing Building or Grounds — Based on observation and record review, LPAs found that the off-limit primary bedroom, bathroom, and garage were being used for child care which poses a potential risk to the health, safety, and personal rights to children in care.

Personal Rights — During observations, LPAs found that several infants were being restrained in devices which were not being utilized for their intended purpose for a period of time such as high chairs, stroller, play pens, and swings which poses a potential health, safety, & personal rights risk to children.

Personnel Records — During record review, LPAs found that all personnel files were either incomplete or missing required documentation. Some staff did not have any files available for review which poses a potential health, safety, and personal rights risk to children in care.

Mandated reporter training — During record review, LPAs found that none of the staff present today had a current Mandated Reporter certificate for Child Care Providers which poses a potential risk to health, safety, and personal rights to children in care.

Family home — home free of safety hazards — During record review, LPAs found that the facility has not maintained a current Facility Roster (LIC 9040) available for review which poses a potential risk to the health, safety, and personal rights to children in care.

Mandated reporter training — Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. During record review LPA observed licensee and assistants Mandated reporter expired on 3/15/24 and 1/16/25. Licensee stated that they assumed it need to be done one time.

Family home — preventive health training, including pediatric CPR and first aid — Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. During Record review LPA observed that licensee and assistant CPR/first aid expired on 1/21/25. Licensee stated that they were planning to schedule a training and Licensee had health issue.

Mandated reporter training — Based on observation and record review the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA observed that the licensee did not have a vaild mandated reporter training on file. Assistant stated that it was missed placed and was not able to find it.
