Personal Rights — S1 forcibly grabbed a child's arm during transition from the playground to the classroom today at 9AM. This poses an immediate risk to the personal rights, health and safety of children in care.

Teacher-Child Ratio — Based on LPA observation and record reviews, the licensee did not comply with the section cited above as LPA observed 15 children with 1 Aide upon arrival. This poses an immediate health, safety or personal rights risk to persons in care.

Teacher Qualifications and Duties — Based on record review and observation, S-1 and S-2 were in Room 1. S-1's transcript was evaluated, but does not have the amount of semester units on it.

Personal Rights — Based on observation, at 10:48AM, LPA observed a staff grab a child by their arm, which poses an immediate health and safety risk to children in care.

Responsibility for Providing Care and Supervision — Based on observation, at 1:45PM, there were two children in Room 2. Staff were walked into Room 1 to get water, then back into Room 2. The door between the room was open. This poses an immediate health and safety risk to children in care.

Teacher-Child Ratio — Based on observation, LPA observed that one staff was with 14 children at 9:43AM and 10:24AM. LPA observed at 10:31AM that staff left 14 children with therapist. At 11:54AM, LPA observed that there was one staff with 16 children. This poses an immediate health and safety risk to children in care.

Limitations on Capacity and Ambulatory Status — Based on interview, the school-age children and preschool children commingling. This poses an immediate health and safety risk to children in care.

Responsibility for Providing Care and Supervision — Based on observation, LPA observed at 10:01AM and 10:46AM that there were children in the restroom. There was no staff positioned where they can visually supervise the children. LPA also observed at 3:05PM that there was a child in Room 2. The staff was standing by the hallway that leads to the restroom. This poses an immediate health and safety risk to children.

Personal Rights — Based on observation, LPA observed that a staff grabbed a child by the wrist and had them sit down on a chair. LPA also observed that a child told to sit down for 10:29AM till 10:37AM. This poses an immediate health and safety risk to children in care.

Criminal Record Clearance — Based on record review, S-1 has cleared criminal record clearance, but fingerprints were no transferred to facility. This poses an immediate health and safety risk to children in care.

Buildings and Grounds — The black wrought iron fence is secured by electrical cords. The green tube slide is not attached to the play structure with appropriate materials. This poses a potential risk to the health, safety and personal rights of children in care.

Personnel Records — LPA observed 2 out of 4 staff files missing immunization records and/ or physician's report and TB test or assessment. This poses a potential risk to the health, safety and personal rights of children in care.

Child's Records — 3 out of 4 children's files reviewed were missing immunization records and a physician's report with TB test or assessment. This poses a potential threat to the health, safety and personal rights of children in care.

Notice to parents that site visit reports are available — 4 out of 4 children's files were missing 2 or more LIC9224 receipts documenting Type A Citations or an Office visit dated: 07/15/2025, 04/15/2025, 10/17/2024, 08/07/2024, 07/25/2024. This poses a potential risk to the health, safety and personal rights of children in care.

Indoor Activity Space for School-Age Children — 7:30 AM LPA observed Director Dinora Sanchez in one classroom with the school age children and preschool children commingled together for drop off until 8:00 AM where there was a total of 10 children in one classroom (5 preschool age and 5 school age). This poses a potential risk to the personal rights, health and safety of children in care.

Teacher/Child Ratio: (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. This regulation has not been met as evidenced by: 3:00 PM LPA observed 13 children and one teacher completing restroom breaks. 3:10 PM LPA observed one teacher and one child inside classroom, leaving the playground with only 2 teachers/1 aide and 36 preschoolers. This poses a potential health and safety risk to children in care.

Napping Equipment — LPA observed 17 new cots, but there were 33 children present today. All mats that were used have holes in them and do not meet the written directives. This is a potential health and safety risk to children inc are.

Teacher/Child Ratio: (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance, except as specified in (b) and (c) below. This regulation has not been met as evidenced by: 9:52 AM - Preschool Teacher in Classroom with 13 preschool children and no Aide. This poses a potential health and safety risk to children in care.

Napping Equipment — Based on observations, the licensee did not comply with the section cited above in 1 out of 30 mats which poses a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observations, the licensee did not comply with the section cited above as the trash can utilized on the playground which is used for food debris did not have a tight fitting lid, which poses a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on observations, the licensee did not comply with the section cited above as LPA observed missing tan bark below slides which poses a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observations, the licensee did not comply with the section cited above 2 slides were not bolted to the ground which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 6 out of 8 files which poses a potential health, safety or personal rights risk to persons in care.

Child Care Center Director Qualifications and Duties — Based on record review, the licensee did not comply with the section cited above in 1 out of 1 which poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in 4 out of 8 staff files did not have a LIC503 (Physician's Report) which poses a potential health, safety or personal rights risk to persons in care.

Personnel Records — Based on record review, the licensee did not comply with the section cited above in 6 out of 8 staff did not have a negative TB Test which poses a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in 2 out of 5 children did not have a negative TB test or TB Assessment on file which poses a potential health, safety or personal rights risk to persons in care.

Sign In and Sign Out — Based on observations and record review, the licensee did not comply with the section cited above as facility is using an electronic PIN system to sign in/out of children without a waiver. This poses a potential health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on observation, LPA observed at 10:41AM and 10:42AM that children were using the sink in the restroom. Staff were not positioned where they can visually supervise children in the restroom. S-4 moved to the restroom during inspection. This poses a potential health and safety risk to children in care.

Drinking Water — Based on observation, children went outside at 11AM. Their water bottles were brought outside at 11:09AM, which poses a potential health and safety risk to children in care.

Buildings and Grounds — Based on observation, there is a metal cabinet that is outside of Room 1 that has mouse dropping inside and on the floor to the right of cabinet. This poses a potential health and safety risk to children in care.

Buildings and Grounds — Based on observation, there were plastic bins in Room 1 that was cracked and used for toys. LPA also observed that the wooden play structure on the left hand side was missing a wooden plank on the top causing there to be a gap in the floor. There is also a plastic play structure that is cracked. Area that is cracked is sharp. LPA also observed that the tarp outside is coming up, which could cause a tripping hazard. This poses a potential health and safety risk to children in care.

Outdoor Activity Space — Based on observation, the tan bark around the plastic structure is low, which poses a potential health and safety risk to children in care.
