Infant Care Teacher Qualifications and Duties — 7:45-8:15 AM C1 was alone in the care of a staff person qualified as an Aide without credits. This poses an immediate risk to the personal rights, health and safety of children in care.

Infant Care Teacher Qualifications and Duties — Based on observation and record review, S-1 and S-2 were with the infant children; neither are a fully qualified infant teacher. This poses an immediate health and safety risk to children in care.

Buildings and Grounds — Rodent droppings observed in the adult women's bathroom at the facility. This poses a potential risk to the health and safety of children in care.

Child's Records — LPA observed C1 & C2's files were missing immunization records and C1's file was missing a Physician's report with TB test results. This poses a potential risk to the health and safety of children in care.

Staff immunization requirements — 3 out of 3 staff files do not have proof of immunization to MMR or DTAP. This poses a potential risk to the health and safety of children in care.

Personnel Requirements — One staff person does not have a negative TB test or TB assessment on file within one year of the employment date. This poses a potential risk to the health and safety of children in care.

Infant Care Activities — Based on record review, the licensee did not comply with the section cited above in 2 out of 3 files which poses a potential health, safety or personal rights risk to persons in care.

Infant Needs and Services Plan — Based on record review, the licensee did not comply with the section cited above in 3 out of 3 files did not have an updated Needs and Services Plan which poses a potential health, safety or personal rights risk to persons in care.

Infant Care Food Service — Based on observation, the licensee did not comply with the section cited above in 2 out of 2 objects as LPA observed two jars with food, no label with children's names which poses a potential health, safety or personal rights risk to persons in care.

Infant Care Food Service — Based on observation, bottles and containers of food brought by the infant's parent were not labeled with the child's name and date. All bottle and food containers are placed in a container that has the child's name. This poses a potential health and safety risk to children in care.

Food Service — Based on observation, LPA observed that there was a bottle of milk and food in the child's container, which poses a potential health and safety risk to children in care.

Outdoor Activity Space — Based on observation, there is no shaded area, which poses a potential health and safety risk to children in care.

Infant Care Center Sleeping Equipment — Based on observation, LPA observed that the sheets in the playyard are not tight-fitted. This poses a potential health and safety risk to children in care.
