Criminal Record Clearance — Based on witness testimony S1 has been providing transportation to children in care and does not have criminal record clearance. This poses an immediate risk to the health, safety and personal rights of children in care.

Criminal Record Clearance — S1 does not have fingerprint clearances and was left alone supervising 12 school age children in the playground. This poses an immediate risk to the health, safety and personal rights of children in care.

Teacher-Child Ratio — Based on observation, the school age children were supervised by unqualified staff. S1, S2 and S3 are aides. This poses an immediate risk to the health, safety, and personal rights of children in care.

Responsibility for Providing Care and Supervision — Based on observation, five school age were left unsupervised in the classroom. This poses an immediate risk to the health, safety, and personal rights of children in care.

Criminal Record Clearance — Based on observation, LPA observed that the volunteer was outside the bathroom with children alone. This poses an immediate health and safety risk to children in care.

Teacher-Child Ratio — Based on observation, there were 17 children with one staff. This poses an immediate health and safety risk to children in care.

Criminal Record Clearance — Based on observation, at 3:25PM, LPA observed that the volunteer was left alone with two school-age children, which poses an immediate health and safety risk to children in care.

Limitations on Capacity and Ambulatory Status — Based on observation, LPA observed that the school-age children were using the grass area and patio area in front of the church and that is in-between the parking lots. Area is not licensed under the school-age license. This poses an immediate health and safety risk to children in care.

Food Service — During LPA’s 07/15/2025 visit, it was observed that there were not enough snacks available for the number of children in care. Only apple slices were later made available. This poses a potential risk to the health and personal rights of children in care.

Food Service — Based on LPA’s observations and interviews conducted, although the menu is posted in the facility, it is not being followed and food on the menu is not available in the facility. This poses a potential risk to the health and personal rights of children in care.

Outdoor Activity Space — Based on interviews and LPA's observations, the facility conducts children’s classes and activities in an outdoor activity area with no fencing and not approved for use by children. This poses a potential risk to the safety of children in care.

False Claims — During interviews with LPA, Licensee provided false statements on 07/15/2025 with regards to the field trip that occurred on 07/09/2025. This poses a potential risk to the health, safety and personal rights of children in care.

Personnel Records — S1, S2 and S3 did not have Personnel Files when requested by the Department. This poses a potential risk to the health and safety of children in care.

Indoor Activity Space for School-Age Children — 7:30 AM LPA observed Director Dinora Sanchez in one classroom with the school age children and preschool children commingled together for drop off until 8:00 AM where there was a total of 10 children in one classroom (5 preschool age and 5 school age). This poses a potential risk to the personal rights, health and safety of children in care.

Admission Policies — Based on interviews, LPA determined children are being transported by a facility vehicle but there is not a clause addressing transportation in the admissions agreement signed by parents in children's files.

Posting of License — Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on observations, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 2 out of 4 files did not have immunization records present which poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in 3 out of 4 staff files did not have a Negative TB test or Assessment which poses a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on observation, LPA observed that grass area and patio area that school-age children were using was not fenced. This poses a potential health and safety to children in care.

Criminal Record Clearance — Based on interview, volunteer spends more than 16 hours per week at the facility, which poses a potential health and safety.
