Child Care Center Director Qualifications and Duties — Based on observation, interview and record review, the licensee did not comply with the section cited above in 1/1 counts, as of 12/22/2025 the CCC does not have a Director which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Records — Based on observation, interview and record review the licensee did not comply with the section cited above in 4/5 staff files were incomplete and Facility Representative had to have main office send documents, seatch for documents via email and have teachers step out of classroom to find/provide documents, which poses/posed a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — Based on observation, interview and record review, the licensee did not comply with the section cited above as they have not yet informed CCL of their change of Director and/or Designee which occured on 12/22/2025, which poses/posed a potential health, safety or personal rights risk to persons in care.
