During inspection, LPA observed C3 sleeping on a portable baby swing inside the room which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, file review and interview, the licensee did not comply with the section cited above in that C3 and C3 did not have a completed LIC 9227 signed by parent and licensee which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, file review and interview, the licensee did not comply with the section cited above in that no documentation of 15 minutes sleep logs were made since April 16, 2926 for C3 and C4 which poses a potential health, safety or personal rights risk to persons in care.
