The glue trap has a warning label that states "Keep Out of the Reach" of children. There were no children in the vicinity of the trap during the observation, therefore this is a Type B citation. However, the accessible trap poses a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on interviews conducted, a staff member aggressively moved a child’s hand from a shelf and using a harsh tone of voice to redirect the child, which is a potential risk to health and safety and personal rights of children in care.
Written directives for drinking water lead testing — Based on records review of required lead testing, the facility had lead values of 5.5 or above on water outlets: Outlet B (33 ppb), and Outlet E (14 ppb). This is a potential health and safety risk to persons in care.
