Staffing Ratio and Capacity — The licensee operated out of capacity on the following dates 09/16/2025, 10/23/2025, 10/24/2025, and 10/29/2025, as demonstrated on licensee's attendance records. This is a potential risk to the health, safety, or personal rights of children in care.
Alterations to Existing Building or Grounds — Based on LPAs' observations, records review and interviews, the home's garage was converted to a living space (daycare room) without permits, which poses/posed a potential health, safety or personal rights risk to persons in care.
Staffing Ratio and Capacity — Based on interviews and records reviewed, on 07/17/2025 there were 13 children present, including four infants, which posed a potentiatl threat to the health, safety, or personal rights of children.
Criminal background check — Based on records review, interviews and observations, assistant #1 was fingerprint cleared, However, assistant #2 was not associated to the facility before she began working for licensee's daycare which poses a potential health, safety, or personal rights risk to persons in care.
Immunizations — Based on interviews and record reviews, Licensee did not have immunization records for 11 out of the 14 children that are enrolled in the facility. The Licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
Alterations to Existing Building or Grounds — Based on LPA's observations, interviews and records review. The daycare storage room and master bedroom were to be “off limits” and were not permitted for childcare use, which poses a potential health, safety or personal rights risk to persons in care.
