Teacher Qualifications and Duties — Based on the observations, interviews, and record reviews, the licensee did not comply with the section cited above as S3 is missing the curriculum course work. S3 not a fully qualified preschool teacher, which posed a potential health, safety or personal rights risk to persons in care.
Criminal Record Clearance — Based on observation, interviews, record reviews, the licensee did not comply with the section cited above with S1's background clearence is not assoicated prior to employment at the facility, which posed a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on observation, interview, record review, the licensee did not comply with the section cited above with no director or a fully qualified Teacher supervising the facility, which posed a potential health, safety or personal rights risk to persons in care.
Carbon monoxide detectors required — Based on observation and interview, the licensee did not comply with the section cited above. LPA did not observed a working carbon monoxide detector in the facility, which poses a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above. LPA observed a rusted nail exposed through composing wood on the corner of the wooden platform around the tree. LPA also observed a small ditch between the cement walkway into the middle of the preschool play yard. This posed a potential health, safety or personal rights risk to persons in care.
