Criminal Record Clearance — Based on records reviewed and information obtained, the licensee allowed Terri Bercasio to act in the capacity of and/or on behalf of the licensee for an undetermined amount of time without a criminal record clearance or a criminal record exemption as required, which poses an immediate health and safety risk to children in care.

Finances — Based on records reviewed, the licensee did not maintain a financial plan necessary to operate the facility, as records demonstrated ongoing financial concerns affecting the facility’s ability to sustain operations and meet financial obligations, which poses an immediate health and safety risk to children in care.

Teacher-Child Ratio — Based on observation and interviews, the licensee did not comply with the section cited above in that one (1) staff member was supervising eighteen (18) preschool children while the other staff member was giving breaks in the infant program and no additional staff were present and available which poses an immediate health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — Based on analyst interviews, the facility did not comply with the section cited above when it had a period of nearly two weeks where two staff members were solely staffing the two preschool classrooms and were left with more than 12 children whenever staff members had to take breaks or assist with meal preparation and no one from the infant program was able to substitute for them which posed an immediate health, safety or personal rights risk to children in care due to the frequency in which the ratio violation occurred.

Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above in that staff (S1's) Mandated Reporter training had expired and not renewed which poses/posed a potential health, safety or personal rights risk to persons in care.

Inspection Authority of the Department — Based on record review, the licensee did not comply with the section cited above, as they did not produce copies of the requested records by the assigned due date, which poses a potential health, safety risk to children in care.

Carbon monoxide detectors required — Based on analyst observation, the licensee did not comply with the section cited above as the infant rooms do not contain a carbon monoxide detector which poses/posed a potential health, safety or personal rights risk to children in care.
