Criminal Record Clearance — Based on records reviewed and information obtained, the licensee allowed Terri Bercasio to act in the capacity of and/or on behalf of the licensee for an undetermined amount of time without a criminal record clearance or a criminal record exemption as required, which poses an immediate health and safety risk to children in care.

Finances — Based on records reviewed, the licensee failed to maintain the financial stability necessary to operate the facility, as records demonstrated ongoing financial concerns affecting the facility’s ability to sustain operations and meet financial obligations, which poses an immediate health and safety risk to children in care.

Infant Care Center Sleeping Equipment — Based on observation and interview the licensee did not comply with the section cited above in that four (4) infants who were sleeping in cribs contained a bottle which poses an immediate health, safety or personal rights risk to persons in care.

Staff-Infant Ratio — Based on analyst observation, the facility did not comply with the section cited above as when analyst arrived in the infant classroom a teacher and aide were supervising nine children putting them out of the regulation required 1:4 staff to infant ratio which poses/posed a potential health, safety or personal rights risk to infants in care.

Infant Care Center Sleeping Equipment — Based on observation and interview, the licensee did not comply with the section cited above in that one (1) infant sleeping in a crib had a pacifier with a clip attached which poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Care Food Service — Based on observation and interview, the licensee did not comply with the section cited above none of the infant bottles contained the current date which poses/posed a potential health, safety or personal rights risk to persons in care.

Inspection Authority of the Department — Based on record review, the licensee did not comply with the section cited above, as they did not produce copies of the requested records by the assigned due date, which poses a potential health, safety risk to children in care.

Staff immunization requirements — Based on analyst record review, the licensee did not comply with the section cited above as staff members #3 and #4 did not have required proof of immunizations which poses/posed a potential health, safety or personal rights risk to children in care.

Personnel Requirements — Based on analyst record review, the licensee did not comply with the section cited above as staff members #3 and #4 did not complete mandatory health screenings, that include a negative TB test result, which poses/posed a potential health, safety or personal rights risk to children in care.

Mandated reporter training — Based on analyst record review, the licensee did not comply with the section cited above as staff members #1 and #5 did not have current Mandated Reporter Training Certifications which poses/posed a potential health, safety or personal rights risk to children in care.

Carbon monoxide detectors required — Based on analyst observation, the licensee did not comply with the section cited above as the infant rooms do not contain a carbon monoxide detector which poses/posed a potential health, safety or personal rights risk to children in care.

Personnel Requirements — Based on analyst record review, the licensee did not comply with the section cited above as staff member #5 did not have a health screening with included cleared TB test result on file which poses/posed a potential health, safety or personal rights risk to children in care.
