Infant C2 was sleeping in an off-limits bedroom with the door closed. The licensee was unabvle to visually observe the infant without moving the door.

Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above in assistant did not have their immunization for LPA to review which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on records review and interview statements, the licensee did not comply with the section cited above in two out of three staff present, which poses/posed a potential health, safety or personal rights risk to persons in care. Staff #2 and Staff #3 did not have a valid mandated reporter training certificate. The licensee stated S2 and S3 need to renew the certificate.
Family home — safe toys and play equipment — Based on observation, the licensee did not comply with the section cited above in 2 of 2 play houses, which poses/posed a potential health, safety or personal rights risk to persons in care. The LPA observed spiderwebs in two playhouses in the backyard. The webs had gathered dry leaves and grassy material and dust showing the webs have been there for some time. The licensee stated the children play outside when it's sunny.
