During record review, Licensing Program Analyst (LPA) observed that licensee was not documenting her fire and disaster drills, to be completed at least once every 6 months.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During employee record review, Licensing Program Analyst (LPA) observed that Assistant #2 was missing Mandated Reporter Training for Child Care Providers.
Immunizations — Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During children's record review, Licensing Program Analyst (LPA) observed that two out of the four children's records that were reviewed were missing updated immunization records.
