Ruiz, Ana Laura Family Child Care

Operating without a license — Based on interview and observation conducted on 06/02/2026, Ms. Ruiz stated she is providing care for three children from three separate families at once, which poses an immediate risk to the health and safety of persons in care.

Personal Rights — Based on interviews and witnesses it was determined that the Licensee pulls the children's hair, roughly pulls their arms when walking and pushes their heads in a aggressive manner. This is an immediate risk to the health and safety to children in care.

Operation of a Family Child Care Home — Based on interviews and witnesses the licensee and helpers has been observed transporting children in care with no carseat. LPA Williams was provided a live photo of a child in the Licensee's vehicle without the use of an appropriate restraint system. This is an immediate risk to the health and safety to children in care.

102416(c) Personnel Requirements The licensee and other personnel as specified shall complete training on preventive health practices, including pediatric cardiopulmonary resuscitation and pediatric first aid, pursuant to Health and Safety Code Section 1596.866. This requirement was not met by: Based on the licensee statement, the helpers do not have files at the facility for the LPAs to review. The licensee stated the helpers do not have CPR/First Aid. This is an immediate health and safety risk to children in care.

102425(b) Infant Safe Sleep-Cribs or play yards shall be free from all loose articles and objects. This requirement was not met by: Based on interviews and a photo, Loose items including a blanket were placed in a portable crib with a child on at least one occassion. This in an immediate health and safety risk to children in care.

Criminal Record Clearance — Based on record review, two adult workers did not obtain the required criminal record clearance or exemption and were not associated to the facility prior to being present at the facility or transporting child. This is an immediate health & safety risk to children in care.

Personal Rights — Based on interviews it was determined that the Licensee forced children to nap by punishing them with timeout. This is a potential health and safety risk to children in care.

Personal Rights — Based on the Licensee' admittance, interviews with daycare children, and daycare parents it was determined that the Licensee was having two daycare children urinate in the backyard. This is a potential health and safety risk to children in care.

102417(g)(10) The home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not be limited to: A baby walker shall not be allowed on the premises of a family child care home in accordance with Health and Safety Code Section 1596.846(b) and (c). This requirement was not met by: Based on interview of and review of pictures and video, the licensee allowed a child to use a baby walker while in care. This is a potential health & safety risk to children in care.

No staff files were available for review. This is a potential health & safety risk to children in care.

Infant Safe Sleep — Based on LPA record review, Licensee admittance and interviews, several infants in care did not have an LIC 9227 on file with the Licensee. This is a potential health and safety risk to children in care.

Family home — home free of safety hazards — Based on record review the Licensee did not maintain a current roster. Licensee was unable to provide a roster when requested. LPAs observed Licensee create a roster during the inspection. LPAs later observed an outdated roster with no current children listed. This is a potential health and safety risk to children in care.

102423(a)(2) Personal Rights Each child receiving services from a family child care home shall have certain rights that shall not be waived or abridged by the licensee regardless of consent or authorization from the child's authorized representative. These rights include, but are not limited to, the following: To receive safe, healthful, and comfortable accommodations, furnishings, and equipment. This requirement was not met by: Based on Licensee admittance, interviews and photos, it was determined the licensee is allowing children and infants to take naps on the couch. This is a potential health and safety risk to children in care.

Personnel Records — Based on the licensee's admittance, interviews and record review, three helpers did not have personnel files. This is a potential health & safety risk to children in care.

102370 (d)(2)Criminal Record Clearance. All individuals subject to a criminal record review pursuant to Health and Safety Code Section 1596.871 shall prior to working, residing, or volunteering in a licensed facility: Request a transfer of a criminal record clearance as specified in Section 102370(j)...This requirement was not met by: Based on record review and the licensees admittance the licensee did not associate the helper before initial presence in the facility. This is a potential health and safety risk to children in care.
