Criminal Record Clearance — Based on record review. During today's inspection LPA Pacheco observed Staff #1 providing care and supervision to children. Staff #1 identified themselves as a teacher. LPA reviewed Guardian and Licensing Information Systems and did not see a fingerprint clearance on file for Staff #1. Director was unable to provide documentation that Staff #1 has been fingerprint cleared which poses an immediate health, safety or personal rights risk to persons in care.
Disaster and Mass Casualty Plan — Based on record review. According to documentation provided by Director, a fire drill was last conducted on 8/31/22. This poses a potential health, safety or personal rights risk to persons in care.
Drinking Water — Based on observation. LPA did not observe a water dispenser in any of the four classrooms. Director stated that staff have to take children's water bottles for children who attend Classrooms A, B & C to be filled out of the faucet in the kitchenette adjacent to Classroom C. Staff in Classroom D have to take children to the water fountain located in the hall way outside of the room to fill their water bottles. This poses a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation. LPA Pacheco observed exposed rusted toilet bolts, a broken sink in the bathroom used by children in Classroom D and a broken sprinkler pipe causing a muddy puddle in the outdoor play area used by Classroom A. This poses a potential health, safety or personal rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on observation. In Classroom A, LPA observed an apple that had been eaten and discarded into a trash can with no tight fitting lid that was accessible to children in care. This poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on observation. LPA observed two staff files that did not contain documentation of required immunizations against measles, pertussis and influenza or flu declination statement which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observation. LPAs did not observe any staff files that contained documentation of a current AB 1207 Mandated Reporter certificare which poses a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on observation. During file review, LPAs did not observe a current pediatric CPR card from either the American Red Cross, American Heart Association or EMSA vendor which poses a potential health, safety or personal rights risk to persons in care.
Teacher Qualifications and Duties — Based on observation. During file review, LPAs did not observe documentation of required units for Staff #1. This poses a potential health, safety or personal rights risk to persons in care.
Personnel Records — Based on LPA observation. During a review of Director’s file, LPAs did not observe documentation of required educational background. This poses a potential health, safety or personal rights risk to persons in care.
