Criminal background check — Based on interview and record review, the licensee did not comply with the section cited above because one helper (S2) who is taking care of the children and one staff (S3) who is preparing food for children daily do not have fingerprinting clearence which poses an immediate health, safety or personal rights risk to persons in care.

Staffing Ratio and Capacity — Based on observation, the licensee did not comply with section cited above because when LPA arrived, licensee is out of home and LPA observed 10 chidren including 2 infants been supervised by one staff which poses an immediate health, safety or personal rights risk to persons in care.

Notice to parents that site visit reports are available — Based on file review, LPAs observed that 3 newly enrolled children(C3, C4 and C6) do not have LIC 9224 in flie which poses a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on dcoumentation, LPA's observed that the Licensee did not consistently check every 15 minutes for the four infants while sleeping, which poses a potential health, safety or personal rights risk to persons in care.

102416.5 Staffing Ratio and Capacity(d) For a Large Family Child Care Home, the maximum number of children for whom care may be provided at any one time when there is an assistant provider in the home, including children under age 10 who reside at the licensee's home and Based on observation, the licensee did not comply with the section cited above: LPA's observed LIcensee and one staff supervising a five(5)infants, upon entry to day care., which poses a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on observation and interview, the licensee did not comply with the section cited above because C1 who is unable to climb out of the crib was sleeping an cot which poses a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on interview and record review, the licensee did not comply with the section cited above because the C1 does not have infant sleeping plan on file which poses a potential health, safety or personal rights risk to persons in care.

(g) The home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not limited to: (9) Each family child care home shall have a written disaster plan of action prepared on a form approved by the Department. All children, age and ability permitting, and the provider, the assistant provider, and other members of the household, shall be instructed in their duties under the disaster plan. As their age and ability permit, newly enrolled children shall be informed promptly of their duties as required in the plan.

Family home — preventive health training, including pediatric CPR and first aid — The licensee did not have current CPR/First Aid training certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Records — The licensee did not have the signed and dated copies of the Notice of Employee Rights, which poses/posed a potential health, safety or personal rights risk to persons in care.

Family home — admission procedures and parent rights — The facility did no have the Notification of Parents' rights Poster, which poses/posed a potential health, safety or personal rights risk to persons in care.

Immunizations — The licensee did not maintain immunization records in chidren's files, which poses/posed a potential health, safety or personal rights risk to persons in care.
