HEALTH RELATED SERVICES: 101226(a):...licensee shall immediately notify the child's authorized representative if the child... sustains an injury more serious ... licensee shall obtain specific instructions from authorized representative regarding action ...taken. Based on the information obtained during interviews, observations, and documentation reviewed it is determined that facility staff did not immediately report the injury of child 1 to responsible party which posed a potential health, safety, or personal rights risk to children in care.
ADMISSION AGREEMENTS: 101229(A): The licensee and the child's authorized representative shall jointly complete a current individual written admission agreement for the child. This documentation shall be maintained at the child care center and shall be available for review. Based on record review the Licensee did not comply with the regulation above as Child 1 has no admission agreement in file which poses a potential health, safety or personal rights risk to children in care.
SIGN IN AND SIGN OUT: The person who brings the child to, and removes the child from, the center shall sign the child in/out. Based on record review the Licensee did not comply with the regulation above as 6 of 11 children in classroom 4 were not signed inor out properly every day in September 2023 as required which poses a potential health, safety or personal rights risk to children in care.
REPORTING REQUIREMENTS: upon the occurrence, a report shall be made to the Department by telephone or fax...next working day...during its normal business hours. In addition, written report... shall be submitted...within seven days following the occurrence... Based on interview & record review Licensee did not comply with regulation above as staff failed to report 9/22/23 incident wherein a CPs report was made regarding child 1 and not reported verbally within 24 hours/in writing in 7 days which poses a potential health, safety, or personal rights risk to children in care.
