The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician... Roster shall be available to the licensing agency upon request. This requirment was not met as evidence by: Based on observation and record review it was determined that the facility owner did not have a full and complete facility roster on hand at the time of LPA's inspection
