Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.2 out of 6 children were missing parent and physician information on the LIC 700.
Infant Safe Sleep — Based on observation, Child 1 (C1) was sleeping in a play yard with an attachment hanging above the crib and toys and a bottle alongside C1. This poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation and file review, C1 was asleep at 9:55 AM when LPA arrived. At 10:10 AM, LPA reviewed sleep log and observed that the log had not been filled out for 8/28/25. C1 woke up at 10:30 AM. Licensee confirmed that she had not completed the log today. LPA provided PIN 20-24-CCP-SP.
Family home — home free of safety hazards — Based on observation, LPA observed a chlorine gallon was on a lower shelf in the laundry room; one has to pass the kitchen to access the laundry room. The kitchen is off-limits and has a safety gate but, it was not locked when LPA arrived. The licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care.

Licensee stated that she has not completed an emergency drill since being licensed.
Infant Safe Sleep — Based on observation, Child 1 was sleeping with a folded blanket as a pillow and had an addition blanket covering the child. There was also an attachment hanging above the crib. The licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. LPA observed that Child 2 was asleep in a baby swing when LPA arrived. Although he woke up shortly after, Licensee stated that Child 2 can only fall asleep on the swing.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. Child 1 has no sleep log; Licensee stated the child was registered 1/21/25. Child 2 has no sleep log since Nov. 2024; Licensee admitted to not completing a sleep log since.
