LPA observed licensee added a day care room to the home and did not notify the Department which poses/posed a potential health, safety or personal rights risk to persons in care.
Alterations to Existing Building or Grounds — The licensee shall provide the Department with a copy of an inspection report when an inspection is required by the local building inspector as a result of the alteration, addition or construction. Based on interview, the licensee did not comply with the section cited above. Licensee stated she is in process of obtaining a permit for addition to the home which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview, the licensee did not comply with the section cited above. Assistant Jennifer Garcia has not completed Mandated Reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above. Child 4, 5, and 7 are missing LIC9227 which poses/posed a potential health, safety or personal rights risk to persons in care.
