Inspection Authority of the Department — on 05/15/26 at approximately 9:42 a.m., LPM and LPA knocked on the front door. At 9:44am, LPM called the licensee’s phone number and left a voicemail. At 9:48am, LPM observed Child #1 alone inside the home. In the rear side of the home, LPM knocked on the glass sliding door and identified and announced herself and no response. At 10:11 a.m., the licensee opened the door after knock and noticed from Salinas PD. This poses an immediate risk to the Health, Safety or Personal Rights to children in care.
Criminal Record Clearance — , interview conducted with licensee. LPM and LPA reviewed Guardian roster dated 06/01/2026 with licensee and verified fingerprint clearance for adults associated in facility. It was revealed that adult, S5, resides in the home and is not fingerprint cleared. This poses an immediate risk to the Health, Safety or Personal Rights to children in care.

Mandated reporter training — Based on record review the licensee did not comply with the section cited above. Licensee Maria and helper Tare were unable to produce proof of renewed training, which poses/posed a potential health, safety or personal rights risk to persons in care.
