Family home — children supervised at all times — Based on interview Maria failed to provide active supervision or take appropriate steps to stop the children’s behavior or prevent a foreseeable injury when she was aware children were jumping on a couch when she exited the room which posed an immediate health, safety or personal rights risk to persons in care as the lack of supervision resulted in a serious injury.
