Child's Records — LPA asked for 4 children's files who attended the facility today. The licensee was unable to provide 3 children's file,which poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — LPA did not observed LIC 9227 in the children (Infant's) file, which poses/posed a potential health, safety or personal rights risk to persons in care.
