Responsibility for Providing Care and Supervision — Based on observation, the licensee did not comply with the section cited above when staff members were gathered together on playground with their backs to children in care. Staff members where not providing 100% visual supervision of children, which poses an immediate health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review the licensee did not comply with the section cited above no staff members had Pediatric CPR and First Aid certificate on file, which poses an immediate health, safety or personal rights risk to persons in care.

Personal Rights — Child was harmed by child care pet.

Personal Rights — Based on record review and teachers’ interviews, one child was injured due to a fallen sliding door. S1disclosed that another child was injured due to a fallen sliding door.

Buildings and Grounds — LPA's observed cleaning solutions, poisons, cat fecal matter, blades, pills, knives, gardening sheers, sharp objects on barbeque, and electrical panel behind unlocked doors, making accesible to children

Food Service;In child care centers providing meals to children, the following shall apply:(1)All food shall be safe and of the quality and in the quantity necessary to meet the needs of the children. Each meal shall include, at a minimum, the amount of food components as specified by Title 7, Code of Federal Regulations, Part 226.20, (Revised January 1, 1990) Requirements for Meals, for the age group served. All food shall be selected, stored, prepared and served in a safe and healthful manner. — No milk was served or observed in any refrigerators during the inspection. Staff was observed changing a soiled diaper with fecal matter and did not wash hands. The same employee was observed coming to kitchen to prepare food without washing her hands.

Teacher Qualifications and Duties — LPA's reviewed 11 staff files. 2 staff did not have emplyee files on site. LPA's observed 2 college transcripts.

Limitations on Capacity and Ambulatory Status — LPA's conducted a census and counted 36 children, facility license is for 35.

Notice to parents that site visit reports are available — LPA learned that facility staff did not provide all families copies of licensing reports documenting a Type A deficiency dated 7/10/25; 8/12/25; 9/2/25; 12/12/25.

Child's Medical Assessments — Based on record review the licensee did not comply with the section cited above when children's files did not contain health assessments, blue cards and required forms which poses/posed a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Failure to report child harmed by day care pet.

Staffing for Water Activities — Based on record review and interview with Facility Representative. Facility staff present at the field trips to the pool did not have proper water safety certifications.

Personnel Records — Staff Files-LPAs reviewed incomplete staff files. 5 out of 11 files were missing health screenings. 8 out 11 files are missing Mandated Reporter Training. 5 files had incomplete immunization records.

Food Service — Facility did not have updated menus posted
