Limitations on Capacity and Ambulatory Status — Based on observation, Interview and record review, the licensee did not comply with the section cited above in the licensee exceeded the licensed capacity of 24, with 26 children observed to be present and enrolled in the school age program and then transitioned 2 of the enrolled school age children to the Preschool program, by commingling the children for both programs, beyond the restricted 1st and last hour of operation requested by the licensee and approved by the department in a waiver; which poses an immediate health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on observation and record review, the licensee did not comply with the section cited above in the licensee was unable to provide proof of health screenings for 2 subject staff present and caring for children which poses a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — Based on observation and record review, the licensee did not comply with the section cited above in the 3 children present did not have complete or missing sign in/out records which poses a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — Based upon LPA record review, the facility did not report an incident occurring on 08/21/24 regarding a staff/child interaction This is a potential health and safety risk to children in care.
Mandated reporter training — Based on (interview) and (record review)], the licensee did not comply with the section cited above in that S4 had expired mandated reporter from 07/23 which poses/posed a potential health, safety or personal rights risk to persons in care.
(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.
