The licensee employed and/or allowed two adult assistants to provide care and supervision of children, at their facility, without a criminal record clearance. This poses an immediate health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above. LPA Lopez observed three infants sleeping in a separate room, with the door closed, and the assistant NOT stationed where the infants could be supervised. This poses an immediate health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above. LPA toured the back yard and observed dog feces in and around the area where children play. However, due to severe weather the children are currently not allowed outdoors. This poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above. The licensee's assistants did not have current Mandated Reporter Training certifications on file. This poses/posed a potential health, safety or personal rights risk to persons in care.
Alterations to Existing Building or Grounds — Based on observation and record review, the licensee did not comply with the section cited above. Licensee was utilizing an off limit room as part of the child care. This poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above. The licensee's assistant (#2) did not have an immunization record on file. This poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above. In reviewing the facility roster, there were names of children that were present at the facility and not listed on the roster. This poses/posed a potential health, safety or personal rights risk to persons in care.
