Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above in that there were 5 disinfectant wipe containers which stated "call poison control" on their label in a cabinet inacceesible to children based on its height. However, this cabinet did not have a key lock or combination lock for the storage area for these 5 itmes. This poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the staff file review the LPA was unable to review a Preventative Health & Safety certificate for at least one staff present.
Child's Medical Assessments — Based on the record review, the Licensee did not meet the above regulation which poses a potential health, risk to the children in care. During the children’s file review the LPA was unable to review an LIC 701 Physician’s Report for C1.
Reporting Requirements — Based on the observation/interview/record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. Upon arrival the LPA was greeted by S2 who introduced themself as the Director. The LPA was informed the Director on file has not been the Director for about a month. LPA observed an LIC 610 Emergency Disaster Plan For Child Care Centers dated 7-25-2025 which lists S1 as the Director.
