Infant Care Activities — Based on the evidence gathered, an infant was observed to be swaddled and sleeping on a boppy pillow during nap. LPA confirmed during the inspection that the boppy pillow used by the infant was in the room.

Limitations on Capacity and Ambulatory Status — , During annual inspection on 4/30/26, LPA observed infants in Room 1 which was not an approved classroom by CCL or the Fire Department. This is a potential health andsafety risk to children in care.

Responsibility for Providing Care and Supervision for Infants — , Based on LPAs observation on 7/7/26, one infant was in the nap room without direct supervision. This is a potential health and safety risk to children in care.

Fixtures, Furniture, Equipment and Supplies — , Based on LPAs observation on 4/30/26 and 7/7/26, nap room did not have adequate lighting for the comfort and safety of infants. This is a potential health and safety risk to children in care.

Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above in all staff had an expired Mandated Reporter certificate which poses/posed a potential health, safety or personal rights risk to persons in care.

Notice to parents that site visit reports are available — Based on the information obtained during documentation reviewed children’s records for C1 – C6 were missing parents or guardians verification they received report of facility Type A deficiency on 11/27/23.

Staff-Infant Ratio — Based on LPA's observation, Room #7 was observed to be out of ratio at 9:43AM with 5 infants and one teacher.

Infant Care Activities — Based on staff interview and photo received it was deteremined that infants were found to be sleeping in swings and boppy pillows for an undetermined amount of time before they were placed on cribs.

Staff-Infant Ratio — Based on LPAs observation, Room #4 was out of ratio when LPAs arrived with 9 infants and 2 staff (Antonio Parker and Noa Iovine).
