This requirement was not met by: Based on observation on 8/26/24, LPA observed napping mats touching/stored together in a preschool classroom which is a potential health and safety risk to children in care.
Personnel Requirements — … Based upon interviews with staff and parents and documents reviewed from the facility and complainant, staff member S1 was alone with 5 children in a classroom and was not visually supervising the other children while changing a diaper (her back was turned), when C1 bit another child (C2) for the second time twenty minutes after the first bite which could have been prevented with adequate staff supervision. This is a potential health, safety and personal rights risk to children in care.
Health-Related Services — … Based upon interviews with Director, staff, complainant and parents, parent of C2 was not notified of the second bite upon pickup from the center. Other parents also stated that notification of injuries and behavior issues (biting) are not consistently communicated or received from the facility which is a potential health, safety and personal rights risk to children in care.
Teacher-Child Ratio — Based on observation, LPA Banas observed 13 children in Room #3 with one staff - Katelyn Salisbury.
Criminal Record Clearance — Based on record review, facility failed to associate 4 staff's livescan fingerprint to the facility - Gabriela Leon (Employed 8/1/23); Rocio Leon (Employed 5/8/23); Katelyn Salisbury and Rouwaida Nouri) (first day 11/13/23)
Personnel Requirements — Based on record review several staff were missing Physicals: Jean Simon; Mary Rodriguez; Even Sabri; Katelyn Elleraas and Sinia Fruge.
