Family home — home free of safety hazards — Based on observation and interview conducted, the licensee did not comply with the section cited above as LPA observed an exersaucer in the home and Licensee disclosed they used the exersaucer for a child (C1) present during water play which poses an immediate safety risk to children in care.

Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above in that cleaning compounds were inaccessible but not locked, which poses/posed a potential health, safety or personal rights risk to persons in care. Items in the bathroom were removed, however, items in the laundry area need to be locked and/or removed.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above in that the crib in use did not have a tight fitting sheet, sheet present was loose which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in that A1 did not have a mandated reporter certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above in that A1 did not have a copy of their measles on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Alterations to Existing Building or Grounds — Based on observation, the licensee did not comply with the section cited above in that LPA observed daycare children in an off limit area (garage), which poses/posed a potential health, safety or personal rights risk to persons in care. Per Licensee the garage is not used for daycare services and children were out their temporarily while Licensee spoke to their spouse. LPA did not observe any immediate dangers.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above in that C1 did not have LIC9227 on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above in that 15 minute sleep checks were not documented for C1, which poses/posed a potential health, safety or personal rights risk to persons in care. A log was present however log only documented started and end time of nap.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in that Licensee did not have a current mandated re[porter certificate on file, which poses/posed a potential health, safety or personal rights risk to persons in care.
