Staff immunization — influenza, pertussis and measles — Based on observation, interview, and record review, the licensee did not comply with the section cited above in not having a current flu vaccination for her self (licensee) which poses/posed a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on observation, interview, and record review, the licensee did not comply with the section cited above in C1 is missing immunization records which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above in an infant was sleeping in a carseat upon arrival to the FCCH which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above in one (1) S1 did not have the mandated reporter training certificate which poses/posed a potential health, safety or personal rights risk to persons in care.
Reporting Requirements — Based on the record review and interview with Licensee, Licensee failed to report the said incident to the department. This poses a potential risk to health and safety of children in care.
