Personal Rights — Rugs in classrooms 126 and 130 with corners and edges curled up which could pose a tripping hazard.
Fixtures, Furniture, Equipment and Supplies — Blue/yellow children sized picnic table/bench with connected benches (picnic table/bench) with seating on both sides of the table in the play yard with seating on one side was observed to not be properly secured due to missing screws.
Personal Rights — S4 indicated that they witnessed S7 and S9 speak inappropriately to children such as yelling. S5 and S6 also indicated that they witnessed S8 speak inappropriately to children such as getting upset and speaking really loud to children. Out of 5 children interviewed, C4 indicated that they witnessed S8 scream at C5.
Pediatric health and safety training (CPR/first aid) — LPA observed that director qualifications were missing from S1’s file and not readily available.
Personnel Requirements — C2 sustained an injury on the hand at the facility and was crying for a good few minutes but Staff didn’t notice and did not attend to C2 until P2 point it out to Staff.
Reporting Requirements — Injury sustained by C1 and C2 were not reported to parent/authorized representative and the department.
Current roster of children — The facility not having a complete roster available for the department. This poses a potential risk to the health and safety of children in care.
Personnel Records — There is no current copy of the Personnel Report (LIC 500) for the department to review. This poses a potential health and safety risk to the children in care.
Disaster and Mass Casualty Plan — Based on record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. No fire drills were documented and not conducted since 02/2025
Fixtures, Furniture, Equipment and Supplies — Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. LPA observed 1 toilet located between class rooms 127 and 130, not flushing properly.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. Staff #4,5,6,7,8, and 9 do not have a Mandated Reporter Training certificate on file.
Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. Staff #1,3,6,7,8 and 9 do not have proof of either or all TB, MMR, TDAP, or the FLU on file.
Child Care Center Director Qualifications and Duties — Based on record review, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. Although a teacher helped the LPA on this date as the Director has been out, there is no Designation of facility responsibility for anyone.
