Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above. While touring the outdoor activity space at 10:26 a.m., LPA's observed four screws on a wooden structure that were not fully intact. The screws were hanging outside the wood approximately 1 inch which poses/posed a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observation, interview, and record review, the licensee did not comply with the section cited. During staff files reviews LPA observed 3 out of 7 files did not have a mandated reporter certification available which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on observation, interview, and record review, the licensee did not comply with the section cited above. During file reviews, LPA observed 2 out of 7 files are missing immunization records and 2 out of 7 files are missing proof of immunization of measles which poses/posed a potential health, safety or personal rights risk to persons in care.
