Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above, as all three staff files were missing immunization records (influenza, pertussis, and measles) which poses/posed a potential health, safety or personal rights risk to persons in care.
Child Care Center Director Qualifications and Duties — Based on record review, the licensee did not comply with the section cited above, as the department was not notified about a director change. Upon record review, LPA Herrera observed an incomplete director’s packet for the new Director. LPA Herrera and Director Nunez reviewed the file together and compiled a list of required documents that must be submitted to Fresno RO by 10/05/2023.
Personnel Requirements — Based on record review, Staff #1 and #3 did not have proof of the health screening, including a test for tuberculosis, performed by or under the supervision of a physician that he or she is in good physical health. Facility representative agrees that she/he will ensure that Staff #1 and #3 obtain the proof of the health screening by his/her physician.
