Written directives for drinking water lead testing — Based on interview and record review, on July 28, 2026, there was a water lead test and the licensee failed to maintain a lead value at or below 5 ppb the Action Level for water lead testing resulting with values of 11 ppb for E- Drinking Fountain Playground (L) and was identified with Action Level Exceedance on the left side bubbler head of the double water fountain was deemed not safe to drink. This is a potential health, safety, or personal rights risk to the persons in care.
Current roster of children — Based on observation, interview, record review, the licensee did not comply with the section cited above in the child/parent roster was incomplete with physician's names and phone numbers which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on observation, interview record review the licensee did not comply with the section cited above in S1- S4 did not have current mandated reporter training during inspection which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on observation, interview, record review , the licensee did not comply with the section cited above in S3 did not have measles vaccination during inspection which poses a potential health, safety or personal rights risk to persons in care.
Responsibility for Providing Care and Supervision — Based on record review and interviews, C1 wandered away from the classroom and was without the supervision of a teacher for approximately 2 minutes. This posed an immediate risk to the health and safety of children in care.
