Teacher-Child Ratio — Based on interview, on at least one occassion, the Director left preschool children with Staff 2, who is qualified as a Teacher Aide resulting in also being out of ratio.
Buildings and Grounds — Based on LPA observation, the preschool classroom has several areas where the walls have chipped paint at the children's level, the wall partition is not secured and wobbly, the wall near the toilets have paint lifted, and toilets are missing screw covers.
Teacher Aide Qualifications and Duties — Based on interview, on at least one occassion, the Director left preschool children with Staff 2, who is qualified as a Teacher Aide.
Personal Rights — Based on interview with Administrator, an inapprpriate conversation occured where staff's voice escalated in an area where day care children were present.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in that Staf #2 did not have proof of taking a mandated reporter since employment, which poses/posed a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in that, LPA observed that both of the windows in the front of the facility were in need of repair. LPA also observed that the window located in the backyard was broken and in need of replacement or repair, which poses/posed a potential health, safety or personal rights risk to persons in care.
