Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above as LPA observed multiple poisons/toxins in a closet which, while inaccessible to children via a sliding latch at the top of the door, was not key locked which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S2 and S3 were missing proof of the Mandated Reporter Child Care Providers Training (AB1207) which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that S1 was missing proof of the measles immunization which poses a potential health, safety or personal rights risk to persons in care.
(a) The licensee shall ensure that each child is accorded the following personal rights: (2) To be accorded safe, healthful and comfortable accommodations, furnishings and equipment to meet his/her needs. Based on interview and record review, the licensee did not comply with the section cited above as a daycare child ingested chemicals after placing a toy in their mouth which poses a potential health, safety or personal rights risk to persons in care.
Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above as LPA observed a toilet in one of the preschool classrooms to be loose and not secured as it was able to be rocked when pushed and LPA also observed that the awning above the play structure had a broken pole hanging from above which poses a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — Based on interview and record review, the licensee did not comply with the section cited above as LPA observed that sign in/out sheets did not all have full signatures from authorized representatives which poses a potential health, safety or personal rights risk to persons in care.
Operating without a license — Based on observation and interview, the licensee did not comply with the section cited above as LPA observed a opened gallon of 2% milk in the kitchen fridge and Licensee stated that the 2% milk was served to children due to being unable to find 1% milk at the store which poses a potential health, safety or personal rights risk to persons in care.
